For Professionals

 
As a professional you take pride in providing quality services and care to your clients with dementia. You set high standards and expectations which you strive to provide to each and every client.

Cognitive Care Coaches can support you in your work and assist you in maintaining your high standards of excellence. We offer a number of services that can support and enhance your care and services.

We begin with a series of cognitive evaluations. These evaluations, offer you an in depth assessment of your client’s current cognitive status and, if applicable, stage of dementia. This information can be invaluable when you are working to create a care plan, selecting relevant care strategies, and/or making life altering decisions regarding your client. In considering both the physical and cognitive needs of your client you will be able to create an all inclusive care plan that will promote success.

Once the evaluations are complete Cognitive Care Coaches can be available to work with you through our consultation services. Through consultation we can confirm your previous assessment results, offer an objective third party assessment, and affirm your current care strategies. In addition, we can offer supplemental and/or alternative care strategies to enhance your current care plan or assist in problem-solving challenging dementia related behaviors.

As a professional, you know the importance of a care plan. You are aware of the need for the direct caregiver to understand, be committed, and skilled in carrying out the care plan. Cognitive Care Coaches can assist you and your caregivers in achieving this goal. Through our coaching services our Coaches work with the direct caregivers, assuring they understand how to execute the care plan and related care strategies. Through education, demonstration, practice, and question/answer time the Coach will work with the direct caregiver to assure your care plan is delivered.

At Cognitive Care Coaches we believe in sharing our knowledge and expertise. Through our Cognitive Care College our Certified Coaches and approved presenters share their “best practices” with professionals serving seniors throughout Minnesota and the Upper Midwest.
