Cognitive Care Coaches:
For Families & Caregivers

Life is a journey. For those with Dementia, the journey can be challenging and a bit bumpy. Unfortunately, there is not single method of care for someone with dementia. There is no instruction manual, cookbook, or guide. As a result, managing the care of an individual with dementia can often be overwhelming.

Do any of these scenarios sound familiar to you?

  • Adult child suspects the parent is struggling to remain independent and safe within their own home but the adult child is unsure of how to confirm this and what to do about it

  • Spouse is having increased difficulty assisting partner with bathing. When attempts are made the partner physically resists

  • Family is unsure if dad should be left home alone

  • Dad used to pride himself in his appearance, now he wears soiled clothes and rarely shaves

  • Mom says that she is doing fine but when visiting, daughter finds spoiled food in the refrigerator and Mom also appears to be losing weight

We begin with an in depth cognitive evaluation we listen, observe and engage the individual with dementia, in a number of everyday tasks, while in the comfort of their own living environment. Through this evaluation process Cognitive Care Coaches are able to identify the individual’s stage of dementia, current abilities, and areas where more assistance is required.

Following the evaluation, our Coaches will provide consultation. Through written information and family/caregiver meetings, our Coaches can explain the results of the evaluation including the abilities and the needs of the individual with dementia. Obtaining this information allows you to plan ahead and make informed decisions In addition, the coach can answer your questions, offer recommendations, and support you as you develop care strategies, determine the individual’s need for supervision, and select appropriate living environments. You’ll gain confidence in care strategies you currently practice and affirmation that you’re doing the right thing for that important person in your life.

If you find that you would like some additional assistance in creating and/or implementing a care plan we can be of assistance to you. Through our coaching services, our Coaches are available to work 1:1 with you and the one you are caring for. We can help you match care strategies to the needs of the individual, teach you how to deliver the care strategies to promote successful outcomes, offer you ideas for additional or alternative care strategies that will enhance current care and decrease or eliminate challenging behaviors.

Because dementia care is evolving, we understand the needs of customers to keep abreast of the latest dementia research, news, and care techniques.
