About Us: Cognitive Care Coaches Cognitive Care Coaches is dedicated to helping people with dementia to Live with dignity. Our clients with dementia continue to work in their gardens, play with their Grandchildren, enjoy the outdoors, and live their lives to the fullest. We have taken a unique approach to providing dementia care and our efforts have been recognized and sought after within the healthcare community. What is unique about our approach and why does it work? Cognitive Care Coaches has re-imagined dementia care. We have moved away from a model of care that promotes Management and Dependency to one that promotes Living and Partnership. In this transition we have experienced great success and have seen first hand how this model of care enhances the quality of life for our clients.
As licensed healthcare professionals our Certified Coaches provide
assistance to
Families and Caregivers
and Licensed Healthcare Professionals. Our Coaches bring both
professional and personal experience to their clients through
evaluation, consultation, coaching , education, and dementia
programming. Though the diseases and conditions that cause dementia present challenges, we believe continued engagement in life is possible.
